Wigan Athletic Community Trust
Progress Health Partnerships designed a bespoke ’12th Man’ programme aimed at promoting health and wellbeing through football. This pioneering programme builds on the pull of professional football to support and educate young people in making healthier lifestyle choices and train participants to take these messages out into the community as “Health and wellbeing champions”

Family Nurse Partnership
The Family Nurse Partnership (FNP) programme is a targeted offer within the 0-5 healthy child pathway, focusing on vulnerable young mothers in their first pregnancy. Progress Health Partnerships were appointed as the Greater Manchester FNP Implementation Manager to support the role out of FNP across the Greater Manchester Area.
The role included the establishment and implementation of local FNP teams in Stockport, Oldham, Rochdale, Salford, Bury, Thameside and Glossop and Trafford; Liaising with midwifery leads to agree potential client notification pathways that meet FNP eligibility criteria and fidelity requirements; Putting in place the infrastructure requirements; establishing, monitoring and reporting on budgets and milestones; ensuring that commissioner, provider and local authority systems support the programme; liaising with other service providers to ensure senior level awareness of FNP and engaging stakeholders.

Healthy Lancashire and South Cumbria: Digital Exemplar Programme
Lancashire and South Cumbria Sustainability and Transformation Partnership (STP) (also referred to as an Integrated Care System (ICS) alongside the Primary Care Transformation Team at NHS England developed the concept of a ‘Digital Exemplar Programme’, to support a primary care approach in enhancing digital capabilities across primary care networks (PCNs).
Progress Health Partnerships were commissioned by Redmoor Health to support the roll out of the programme to enable GP practices to work more efficiently as well as bringing real benefits for patients and supporting practice teams to further develop their capability and capacity in providing different digital modes for delivery of care.
